I read this comment from an FC4 Mod enthusiast's post over on the Nexus site:
Any idea where something which could create this kind of scenario might reside, JR? It looks like a worthwhile addition if it's possible.

I remember the same idea being discussed in Ziggys forum, but AFAIK it is not possible. I would (of course) be very interested in such a scheme.

I would love to do this, but it doesn't seem possible at the moment.
Maybe if we ever figured out the lua scripts and we found a script that handled player spawning we could shoehorn in something that reduced some values (xp, cash, karma etc), but even that might be quirky since for all we know the same script is used whenever a player loads a new mission area, leaves a safe house or starts a new game. Unless there is a separate script that handles dying...
I kinda doubt these sorts of events are tracked through the lua scripts though. Seems more like core gameplay stuff that would be handled through the main .exe
